 Transitional care and day respite services are provided for the elderly in need, flexibly accommodating a variety of family needs. To provide holistic care for the elderly in need of short-term convalescence, caregivers are also given the opportunity to rest, helping them to continue to live a life with faith, hope, love and flourishment in their caregiving journey.

Holistic Development

 

Case managers and professional teams will conduct individual holistic health assessment for each individual and design diversified services and activities according to their cognitive, rehabilitative, psychological, social and spiritual needs, as well as their interests and backgrounds. The elderly are encouraged to establish a regular daily schedule to reduce confusion and anxiety, especially for those with cognitive impairment and dementia. We provide round-the-clock training to slower the brain function deterioration.

Individual Care Plan

 

We respect the uniqueness and preferences of each elderly, and value their autonomy and independence, privacy and dignity. We also work closely with family members, church pastors and multi-disciplinary colleagues to develop a personalized and holistic individual care plan, in order to comprehensively address physical, mental, social and spiritual needs of the elderly.

Rehabilitation Services

 

Registered physiotherapists and occupational therapists provide rehabilitation assessment, treatment services according to their individual abilities and needs, such as stroke rehabilitation, weight and pain management, training of activities of daily living, cognitive training, swallowing training, acupuncture, etc., as well as the use of advanced rehabilitation equipment to maintain or enhance their physical activity and independence.

Spiritual Art Activities

 

The integration of art and spirituality and the utilization of various elements of expressive arts to enhance the holistic well-being of the elderly. We have a “Jehovah Rapha”   Art and Spiritual Garden and we create an aesthetic environment with art works in the sitting room and dinner room, where the elderly could try to connect with expressive arts and improve their quality of life conveniently and comfortably.  The “Jehovah Rapha” Spiritual corner, the “Jehovah Roi”   Silent Prayer Corner and the Hall enable the elderly to purify their souls. There are also art works and bible verses hanging all around to create a quiet, pastoral and comfortable environment for the elderly. Also, there is an  “Immanuel" Garden for spiritual retreats, flower viewing and gardening, where the elderly can enjoy the integration with nature and nurture their spirit as well.

Pastoral Spiritual Care

 

We have daily Morning Prayer and regularly organize various spiritual care activities, including prayer meetings, Retreats, Bible Study Groups as well as pastoral care and bedside care etc. , to facilitate the establishment of a spiritual support network for the elderly. On every Sunday, we have service and preaching by the priests.  We encourage the elderly and their families to participate together to enhance the care and pastoral care of the elderly.

Transitional Care Services
  • Suitable for the elderly whose family environment has changed
    Specially for the newly discharged / frail / stroke elderly, under the care of our multi-disciplinary team, the elderly will undergo a targeted rehabilitation program and prepare for going back home together with their families.

  • Suitable for the elderly whose body function has changed
    Specially for the newly discharged / frail / stroke elderly, under the care of a multi-disciplinary team, the elderly will undergo a targeted rehabilitation program and prepare for coming back home together with their families.

  • Check-in conditions
    Transitional accommodation for a minimum stay of 14 days or more and less than one month is charged on a daily basis.

Day Respite Service
  • Suitable for those who need minimise the caregiving burden
    Day respite service supports and shares the responsibility of caregivers in caring for the elderly on a long-term basis, giving them the opportunity to take a short break.

  • Extended hours services
    We provide extended hours services to support the caregivers and release their caregiving burden. 

  • Length of stay
    The minimum stay for day respite service is 14 days

You may consider arranging a temporary stay in our Kwong Yum Care Home where the elderly could have an individual care plan with the advice of a physiotherapist, occupational therapist, speech therapist, nursing staff and a geriatrician. After that, you can decide whether to return home according to the progress of the elder's recovery.

Our professional team will prepare a plan for the elderly to age at home, and we understand that this is a very challenging task for caregivers with little experience.

 

Our occupational therapists can visit the elderly's home to conduct risk assessments and provide suggestions for improvements, such as installing handrails in toilets, purchasing shower chairs, commode chairs, etc., to help create a suitable living environment for the elderly.

 

In addition, we have training programs for caregivers to learn the proper way to care for the elderly, so that they can continue the quality care as they receive in our care home.
